DPN Check is a small point-of-care device that stimulates and records signals from the sural nerve — a sensory nerve that runs along the outer ankle and lower calf — using surface sensors placed near your ankle and foot.

The device delivers a mild electrical stimulus, which most patients describe as a brief tapping or tingling sensation rather than pain, and measures how the nerve responds. A result is available in 30–60 seconds.

No special preparation is needed. You don't need to fast, and there's no need to stop your usual medications before the test.

Understanding SNAP amplitude and nerve conduction velocity

SNAP stands for Sensory Nerve Action Potential. Its amplitude reflects how many healthy sensory nerve fibers are responding to the test — in simple terms, a stronger signal generally suggests more intact nerve fibers, while a smaller signal can suggest fewer fibers are conducting normally.

Nerve conduction velocity measures how quickly that signal travels along the nerve. Slower conduction can point to changes affecting the nerve's insulating layer, while reduced amplitude more often reflects loss of the nerve fibers themselves — diabetic neuropathy can involve either pattern, sometimes both.

Both legs are tested because diabetic neuropathy typically affects the longest nerves first and tends to progress symmetrically from the feet upward. Testing both sides helps confirm whether a pattern is consistent with diabetic neuropathy, rather than a one-sided problem from an unrelated cause, such as local nerve compression.

Your results are compared against expected reference ranges and are always interpreted alongside your symptoms and clinical exam — never in isolation. Broadly: results within the expected range are reassuring; borderline results may prompt closer monitoring or repeat testing; and results outside the expected range, especially alongside consistent symptoms or exam findings, support a neuropathy diagnosis and shape how actively we manage it.

Clinical examination

Nerve conduction testing complements, and never replaces, a hands-on examination. At each visit where neuropathy is a concern, I assess:

  • Sensation, using a monofilament to check protective sensation at standard points on the foot
  • Vibration sense, using a tuning fork — often affected early in diabetic neuropathy
  • Ankle reflexes, which can diminish or disappear as neuropathy progresses
  • Circulation, by checking pulses in the feet and looking for signs of reduced blood flow
  • Skin condition, including dryness, calluses, and any breaks in the skin
  • Foot shape and structure, looking for deformities that increase pressure points and ulcer risk
  • Any existing wounds or ulcers, and signs of infection such as warmth, redness, swelling, or discharge

Recommended supporting tests

Not all neuropathy is caused by diabetes alone, and some contributing causes are reversible or independently manageable. Depending on your history and findings, I may recommend additional testing, including:

  • Blood sugar control (HbA1c) — poorly controlled glucose is the single biggest driver of diabetic nerve damage
  • Vitamin B12 levels — B12 deficiency causes a nerve-damage pattern that can resemble diabetic neuropathy and is often reversible with supplementation
  • Thyroid function — both underactive and overactive thyroid disease can contribute to nerve symptoms
  • Kidney function — chronic kidney disease can independently affect nerve health and changes how some medications are dosed
  • A review of alcohol use and current medications, both of which can contribute to nerve damage independent of diabetes
  • Vascular assessment of blood flow to the legs and feet, since reduced circulation can compound nerve-related foot risk
  • Additional or more detailed nerve testing, in select cases, if the pattern of symptoms or findings doesn't fit typical diabetic neuropathy alone
